Dual Diagnosis Lead - Milestone / Peckham Rye - internal applications only

Working in partnership with Southwark Forensic Mental Health Team, you will be supporting service users, in two supported living residential units. The individuals you will be working with will be men, with a forensic history, with mental health issues, and additional complex needs - including substance misuse, housing issues, and learning disabilities. 

We design a support package with goals that evolve with people’s changing needs and wishes. We offer high quality support, to help people take control, improve the quality of their lives and face the future with confidence. No matter how complex a person’s needs, whether they’ve tried support services or not in the past, we can provide person-centred support that helps individuals decrease isolation, increase independence, learn new skills, integrate into the community and make the most of every new opportunity.

The typical working hours for this role are Monday to Friday 9am - 5pm.

Role Responsibility

To promote integration and ensure service users with a dual diagnosis do not fall between the gaps of services:

  • To build collaborative partnerships with Drug and Alcohol Recovery services within Southwark enhancing care pathways for those with a dual diagnosis.
  • Facilitate joint working arrangements across the borough between the Southwark Forensic Mental Health team and Drug and Alcohol Recovery services to ensure that individuals with a dual diagnosis do not fall through the net of services, by identifying, monitoring and responding to the needs of this group.
  • Work collaboratively to assist individuals with a dual diagnosis to engage effectively with services provided in the borough of Southwark.
  • Work alongside local substance misuse hubs, to monitor care pathways through regular attendance at case meetings, establishing joint working arrangements on a case by case basis, setting up shared support plans that will enable individuals to engage with the holistic support, to achieve greater recovery.
  • Identify, assess and coordinate the longer-term support needs of individuals with a dual diagnosis and other associated complex needs with the aim of facilitating improved outcomes for this client group.
  • Ensure that service users have individually tailored exit plans and follow up plans for each individual according to their needs. This process will provide a safety net in case personal circumstances have deteriorated after support has ended.
  • Co-ordinate and deliver staff training and development in relation to dual diagnosis, within Milestone, Peckham Rye and whenever possible, other Turning Point services.

The Ideal Candidate

Essential:

  • proven experience in a similar role along with a good awareness of dual diagnosis and complex social issues or completion of a relevant degree
  • being flexible, patient and non-judgemental
  • good IT and Communication skills

Desirable:

  • level 3 NVQ in social care
  • completion of a relevant degree
