Support Plan Coordinator - Salford

We currently have an opportunity for a Support Plan Coordinator to work across our Salford services, where we support adults with learning disabilities and complex needs.
Role Responsibility
Working on a fixed term basis, over 3-4 months, you will have the opportunity to make a big impact over a short period, by supporting with specific tasks across the services. We are looking for somebody who has the ability to hit the ground running, supporting with the coaching and development of support plans across our services. Building on your previous experience in this area, and of working with people with learning disabilities, you’ll develop and coordinate support plans, alongside the people we support, current staff and families. Coaching will play a big part in this role as you will also have the responsibility of mentoring the staff teams in the creation and delivery of comprehensive support plans.
The Ideal Candidate
What skills and qualities do I need to have?
- Experience of working with individuals with autism, learning disabilities and communication difficulties
- A track record of supporting and empowering people to live more independently in their community
- Knowledge and experience of developing and writing care plans
- Previous experience in coaching and mentoring staff
- NVQ level 3 in health and social care.
- You’ll also ideally have a full driving license and access to a car.
