Job Introduction

Raising the bar for people-centred outreach services, you’ll enable people to learn the skills and knowledge they need to understand and manage their own situation more independently. No matter how complex someone’s mental health needs, you’ll provide support that helps individuals to decrease isolation, integrate into the community and make the most of every new opportunity.

Role Responsibility

Hertfordshire Community mental Health Services provides support to adults over the age of 18 with mental health issues and complex needs, including people affected by drug and alcohol misuse, those with a learning disability and their carers and families. We visit people in their own homes as well as a residential service or meet at community locations across the area and working together, we design a support package with goals that evolve with people's changing needs and wishes.  We offer high quality support, to help people take control, improve the quality of their lives and face the future with confidence.  No matter how complex a person's needs, whether they've tried support services or not in the past, we can provide person-centred support that helps individuals decrease isolation, increase independence, learn new skills, integrate into the community and make the most of every new opportunity.

Every individual is unique and every person requires support that’s purpose-built for them.

  • We can provide individually tailored community support to individuals recovering from mental health difficulties, ensuring that individuals are fully involved in deciding what support they would like, when they would like it and from whom.
  • We can cater for a wide variety of support needs and will tailor our package of support to the needs of each individual by fully involving service users in creating their own support package.
  • We know that different people will need and want different kinds of support and so we will provide a flexible and service user led service to ensure that we respond to each person in the way that is most helpful to them.
  • We can adjust our packages of support to reflect differences in areas of support, frequency of support and duration of support. We will work with individuals to create a person centred, recovery orientated support plan and work alongside individuals enabling them to learn the skills and knowledge that they need to understand and manage their own personal situation more independently in the future.
  • All support is tailored to the needs of the individual but below is a summary of the main areas of support that our outreach teams can provide:
