Recovery Worker - CCA Lead - City & Hackney

Job Introduction

Right now, we are now looking for a talented hospital liaison recovery worker to work within our community-based substance misuse services in the City of London and Hackney.

The service is delivered in partnership with Mind CHWF and London Friend and there is a focus on partnership working throughout the service to support those in the local community who need assistance to achieve their goals. We deliver treatment and support across the neighbourhoods ensuring a holistic and inclusive approach to recovery in this service.

As a Recovery Worker Care Act Assessments Lead you’ll have the chance to make a real difference to the lives of the people we support who are experiencing difficulties from drugs and alcohol use, working with them to develop the personal, flexible and realistic support packages they need to help turn things around.

Role Responsibility

To assist in delivering Turning Point’s Substance Misuse strategy by planning and providing high quality, innovative care which reflects our person centred values and the high levels of ambition we have for the recovery of the individuals for whom we provide support.

Key Responsibilities:

Lead on coordination of care act assessments across the service, liaising with Adult Care Services. The role will:

  • Complete CAAs
  • Provide training and support for recovery workers to complete CAAs
  • Upload completed assessments to Mosaic
  • Support the set up and facilitation of PSI groups relevant to Tier 4
  • Ensure robust aftercare plans are in place
  • Arrange Tier 4 placements
  • Updating Tier 4 trackers
  • Attend Tier 4 panel

The role will also manage a caseload of service users and coordinates their care. Work flexibly in a variety of settings to create care and support plans that meet the needs of the service and clients in accordance with the recovery agenda.

The Ideal Candidate

Essential Requirements:

  • Substance misuse knowledge 
  • Recovery caseload management  of both high volumes and complex nature
  • Able to deliver client interventions in a person- centred way
  • Demonstrable skills and knowledge in assessing risk presented by clients to themselves and others
  • Harm reduction, suicide and self-harm awareness
  • Wide and flexible range of client interventions
  • Management of incidents of a violent or aggressive nature
