Housing Support Worker - Peer Support Service

Job Introduction

Turning Point and Hertfordshire Mind Network effectively work in partnership to deliver the County wide Peer Support Service. The service provides support to adults with mental health and other social needs including housing issues. We offer high quality support, to help people take control, improve the quality of their lives and face the future with confidence. No matter how complex a person’s needs, whether they’ve tried support services or not in the past, we can provide person-centred support that helps individuals decrease isolation, increase independence, learn new skills, integrate into the community and make the most of every new opportunity.

You will work with the public and private sector organisations, including local Councils, Housing Associations and other Housing providers to develop the housing service to deliver positive outcomes to service users and carers. You will provide housing advice to service users and carers, with the flexibility to offer practical assistance to service users to regain and maintain control over their lives through the peer support service. You will be part of a diverse and established team based within Hertfordshire Mind Network, where you will support referral processes and general administration and day to day operations.

The working hours are between 8:30 / 9:00 - 17:00 Monday - Friday.

This is a part-time role - 24 hours per week.

Role Responsibility

The key accountabilities of the role are:

  • Developing, in consultation with Service Users, flexible and realistic support packages/person centred plans within agreed guidelines or service models.
  • Providing 1:1  solution focused practical support including outreach and office based & telephone support
  • Light touch Peer Support (The service has dedicated Peer Support Workers to provide in-depth peer support
  • Active Referrals/Signposting – working with individuals to identify resources/services to meet their needs and providing appropriate support to access these effectively
  • Crisis Planning & management – promoting self-management through individuals building an understanding of their own triggers/coping strategies.
  • Ensuring that a collaborative approach is used, with effective communication links with external professional groups e.g.  Councils, Housing Associations, Night shelters, Temporary accommodations resources, GPs, Social Services, etc. and to work as an effective member of any multi-disciplinary team.
  • Ensuring record keeping is maintained to the required standard at all times and contributing to service monitoring requirements.
  • Undertaking responsibility for clinical risk and needs assessment and the formation and implementation of risk management plans.

The Ideal Candidate

Essential:

  • proven experience in a similar role along with a good awareness of mental health and housing problems or completion of a relevant degree
  • a full UK Driving Licence and access to a vehicle
  • being flexible, patient and non-judgmental
  • good IT and communication skills

Desirable:

  • housing legislation knowledge
  • experience of working directly with Council and Housing Associations
  • level 3 NVQ in social care
  • completion of a relevant degree
  • good understanding of mental health /complex needs and impact on wellbeing
