Female Mental Health Support Worker - Douglas House Outreach

Job Introduction

Raising the bar for people-centred outreach services, you’ll enable people to learn the skills and knowledge they need to understand and manage their own situation more independently. No matter how complex someone’s mental health needs, you’ll provide support that helps individuals to decrease isolation, integrate into the community and make the most of every new opportunity.

You will be supporting clients in the community, encouraging them to engage in drop – in activities in community centres, attend medical appointments, and encourage/ support people to engage in social inclusion activities. You will work with clients in their own homes, ensuring they are supported in taking medication, assisting them in activities of daily living, and provide emotional support if necessary, reporting any concerns to the Outreach Manager.

The role involves rotational shifts including evenings, weekends and bank holidays.

Role Responsibility

You will be supporting clients in the community, encouraging them to engage in drop – in activities in community centres, attend medical appointments, and encourage/ support people to engage in social inclusion activities. You will work with clients in their own homes, ensuring they are supported in taking medication, assisting them in activities of daily living, and provide emotional support if necessary, reporting any concerns to the Outreach Manager or the Nurse Manager.

Working with qualified nurses as an Outreach Support Worker, you’ll recognise indicators of deteriorating mental health and help to implement interventions. Promoting independence, well–being and healthy life choices, you’ll also get involved in various activities – from supporting daily living skills following rehabilitation that could include walks, theatre trips, gym sessions and building social networks for clients in the community.

The Ideal Candidate

We are ideally looking for an Outreach Support Worker with an NVQ, experience of working with people with long–term mental health issues and a solid understanding of the issues they face and the recovery model. A great team player with lots of energy, you’ll also need the strong communication skills, flexible approach and positive attitude needed to achieve targets in a challenging, changeable environment. With empathy, understanding and patience, we’ll not only expect you to love talking to people – but be passionate about helping them too.

Having a driving licence is highly desirable.
